This is an app for administering the KCCQ 12 digitally. Traditionally, PDFs have been used which make it less accessible. The Kansas City Cardiomyopathy Questionnaire (KCCQ) is a well-validated and widely used instrument that measures patient-reported symptoms, function and quality of life for patients with heart failure. This is the 12 part shortened version that has been converted to a digital form for easier administration.
